It is not unusual that during your programme of study, certain circumstances of a medical or personal nature can adversely affect your academic performance.  If you feel this to be the case, then you might wish to complete a PMC form.

However, before submitting a PMC form, make sure you have read the guidance notes on the Personal Mitigating Circumstances procedure.  You  will find this in your programme handbook or on the University's website here.

The guidance notes will give you details as to when the form should be submitted.  Take particular note of the University's policy on late submissions.  For each assessment affected, be it an examination, coursework assignment, presentation etc.  specify whether you are claiming mitigation for :

  1. Standard of performance
  2. Late submission of assessed work or
  3. Non-submission of assessed work / non attendance at an exam.

Please note that you can only claim for one of these for each assessment.

The PMC form

If you decide to submit a PMC form, make sure you complete it yourself and all the five sections.  The form can be dowladed from here or obtained from your University handbook, School office or Student Life desk in University House, Allerton or Adelphi buildings. 

If several of your modules/components are affected by one set of circumstances, you can list all the modules/components on the one form.

It is important that you attach all documentary eveidence to support your PMC form.  However do not assume that if you attach evidence e.g. a doctor's note or a death certificate etc it will be sufficient to support your case.  You need to explain in some detail (see section 4 of the PMC form) how the mitigating circumstances affected your work - this might also include preparation of the assignment/exam/presentation etc.  In short, circumstances, events or accidents alone will not guarantee acceptance of the PMC.  It is the evidence which a) confirms the event and b) explains the effect of the event on you.  Finally, make a copy of the completed form and submit it to your School office.  As the School to sign it for you as a receipt.

Remember that if a PMC is rejected, you can not resubmit it with more evidence.

Student Support Plans

Students who have a support plan in place under the AoN procedure or ad hoc arrangements made, must not submit a PMC for the matter already covered.  However, if you are experiencing additional personal mitigating circumstances which could not have been anticipated or the worsening of the symptoms covered by the student support plan, then you could complete a PMC form.
